Comprehensive Support, Every Step of the Way
Clarity Across the Safety Life-cycle
From early planning to field validation, our assessments and audits cover every phase of the safety life-cycle. We help identify gaps, close compliance loops, and build confidence in your SIS program.
Design-Phase FSAs
We assist with FSAs 1 through 3, guiding the team from H&RA through SIL verification and into commissioning with confidence.
Operational FSAs
Post-startup FSAs validate implementation, proof testing, and ongoing compliance under IEC 61511.
Functional Safety Audits
Our audits offer a broad view of your program's performance, supporting internal and regulatory requirements.
Trusted, Standards-Based Evaluations
Whether you’re in design, commissioning, or operation, a Functional Safety Assessment (FSA) or audit is your opportunity to verify that your Functional Safety Program and Safety Instrumented System (SIS) meets its intended risk reduction and documentation requirements. We offer both FSAs and broader audits to support projects and ongoing operations, tailored to where you are in the safety life-cycle.
Our evaluations are more than just checkbox exercises. They’re structured to uncover meaningful insights, build confidence, and promote accountability across teams. We clarify what’s complete, what needs improvement, and how to move forward with full traceability.
Our work is grounded in IEC 61511 and IEC 61508 — with a practical understanding of what it takes to apply these standards in the real world. From field validation to boardroom strategy, we provide the technical rigor and clarity needed to stay compliant and stay on course.

FSA Stage 1, 2 or 3 – Design Stage
Design-Phase FSAs
We provide third-party Functional Safety Assessments (FSAs) during the design phase to confirm your safety life-cycle is on track. These include FSA 1, conducted after the Hazard and Risk Assessment (H&RA) and before SIL selection; FSA 2, after SIL determination but before detailed design; and FSA 3, once the design is complete and before commissioning.
Get a Quote
Each assessment includes a review of your functional safety plan, Safety Requirements Specifications (SRS), HAZOP (Hazard and Operability Study), and LOPA (Layer of Protection Analysis) to verify alignment, traceability, and risk reduction intent.FSA Stage 4 or 5 – Operational Stage
Operational FSAs
We also perform later-stage FSAs to confirm that your SIS was implemented as designed and is still performing as required. FSA 4 occurs after commissioning and before startup. FSA 5 is a periodic review during operation to ensure continued compliance.

These reviews focus on proof test records, maintenance logs, change management, and open risk gaps. At SIL Safe, our goal is to ensure your SIS remains aligned with IEC 61511 and delivers the intended level of protection.Functional Safety Audits for Existing Facilities
Functional Safety Audits
In addition to formal FSAs, we offer audits that deliver a broad view of your functional safety program. These are ideal for clients preparing for internal or regulatory audits or looking to review performance across the full safety life-cycle.
Get in Touch
Audits often examine documentation traceability, PFDavg calculations, proof test coverage (Cpt), team competency, and adherence to safety lifecycle requirements. We tailor our review to your program goals and operational realities.
Common Questions About FSAs & Audits
Here are some of the most frequent questions we hear from clients exploring assessments or audits for their functional safety programs.
What's the difference between an FSA and a functional safety audit?
An FSA (Functional Safety Assessment) is a formal verification step tied to specific stages in the safety life-cycle—typically design, installation, and commissioning. Broadly speaking, FSA 1, 2, and 3 are used during the design, construction and testing phases of the safety life-cycle. A functional safety audit is intended for operating facilities. It assesses if a facility is doing what their procedures say.
When should we perform FSA Stage 1?
FSA 1 is performed after the HAZOP and risk assessment, but before SIL selection. It ensures that the foundation for the SIS design is traceable and technically sound. Some companies split FSA Stage 1 into multiple sub-stages as well.
Can you help us prepare for a regulatory inspection?
Do you offer assessments for facilities outside the U.S.?
Absolutely. We support global clients through a mix of virtual engagement and in-person site visits depending on location and project needs.
