  • FSA Stage 1, 2 or 3 – Design Stage

    Design-Phase FSAs

    We provide third-party Functional Safety Assessments (FSAs) during the design phase to confirm your safety life-cycle is on track. These include FSA 1, conducted after the Hazard and Risk Assessment (H&RA) and before SIL selection; FSA 2, after SIL determination but before detailed design; and FSA 3, once the design is complete and before commissioning.

    Each assessment includes a review of your functional safety plan, Safety Requirements Specifications (SRS), HAZOP (Hazard and Operability Study), and LOPA (Layer of Protection Analysis) to verify alignment, traceability, and risk reduction intent.

  • FSA Stage 4 or 5 – Operational Stage

    Operational FSAs

    We also perform later-stage FSAs to confirm that your SIS was implemented as designed and is still performing as required. FSA 4 occurs after commissioning and before startup. FSA 5 is a periodic review during operation to ensure continued compliance.

  • What's the difference between an FSA and a functional safety audit?

    An FSA (Functional Safety Assessment) is a formal verification step tied to specific stages in the safety life-cycle—typically design, installation, and commissioning.  Broadly speaking, FSA 1, 2, and 3 are used during the design, construction and testing phases of the safety life-cycle.  A functional safety audit is intended for operating facilities.  It assesses if a facility is doing what their procedures say.    

  • When should we perform FSA Stage 1?

    FSA 1 is performed after the HAZOP and risk assessment, but before SIL selection. It ensures that the foundation for the SIS design is traceable and technically sound.  Some companies split FSA Stage 1 into multiple sub-stages as well.

  • Can you help us prepare for a regulatory inspection?

    Yes. We provide pre-audit reviews and documentation walkthroughs to help facilities get ready for OSHA, EPA, or internal compliance checks.

  • Do you offer assessments for facilities outside the U.S.?

    Absolutely. We support global clients through a mix of virtual engagement and in-person site visits depending on location and project needs.
